Appendix 2
Guidance on Pre-Race Physical Examinations for Participating Athletes by the CTSA
1. Necessity of Physical Examination
(1) Triathlon sports involve long distances, high intensity, and unstable environmental conditions. The stimulation to the human body is intense and profound, leading to various acute injuries that are inevitable. The main focus of these injuries is on the cardiovascular, respiratory, central nervous, gastrointestinal, and musculoskeletal systems. Severe cases can be life-threatening.
(2) Due to the unpredictable nature of acute injuries, comprehensive physical examinations, symptom monitoring, active prevention and treatment of injuries, and establishment of health records are essential and necessary for athletes. Good physical condition is the foundation for engaging in triathlon sports. Participating in triathlon events while injured increases the health and safety risks for athletes.
(3) In order to ensure the health and safety of participating athletes to the greatest extent possible and establish a proactive prevention mindset, these guidelines for physical examinations have been developed. Athletes who fail to undergo the required health check or whose examination results do not meet the following requirements are not allowed to participate in the event.
2. General Requirements
(1) Pre-Race Physical Examination
Athletes must promptly understand their own health and physical condition and, prior to the race (within a maximum of six months before the race), undergo a physical examination at a hospital of “Level II Class A” or above.
(2) Self-Monitoring
A normal result from a single pre-race physical examination only indicates the health status reflected by the selected examination items. Due to limitations in examination methods and items, it cannot completely eliminate potential risks in the body. Moreover, due to the “variability of human physical conditions,” it cannot prove that athletes are fully capable of safely completing the race. Therefore, each participating athlete must:
A. Proactively ensure good health conditions before the race, without symptoms such as cold, fever, viral or bacterial tonsillitis, pharyngitis, gastroenteritis, and other acute or chronic injuries.
B. Athletes should constantly monitor their own physical condition during the race. If symptoms such as chest tightness, chest pain, palpitations, nausea, breathing difficulties, abnormal fatigue, or other abnormal conditions occur, they should immediately stop the race and seek assistance from the nearest medical point or event staff.
3. Physical Examination Items
The following physical examination items are designed based on common acute injuries in triathlon sports:
(1) General Internal and External Medicine Examination (for all athletes)
Key inquiries and differentiation for participating athletes include the following conditions:
A. Cardiovascular diseases, such as hypertension, valvular diseases, coronary heart disease, and cardiomyopathy.
B. Central nervous system diseases, such as epilepsy and severe neurasthenia syndrome.
C. Family history of sudden death.
D. History of exercise-related stress or acute injuries.
E. Respiratory system diseases, such as allergic bronchial asthma, acute or chronic respiratory infections.
F. Gastrointestinal diseases, such as active stage of digestive ulcers.
(2) Blood Pressure Examination (only for athletes aged 13 and above)
Athletes under 50 years old: Systolic pressure ≤ 130 mmHg (17.3 kPa), Diastolic pressure ≤ 85 mmHg (11.3 kPa).
Athletes 50 years old and above (including 50 years old): Systolic pressure ≤ 140 mmHg (18.6 kPa), Diastolic pressure ≤ 90 mmHg (12 kPa).
(3) Electrocardiogram (ECG) Examination (for all athletes)
ECG reports must be diagnosed by a physician, and the evaluation criteria are as follows:
A. Normal ECG (confirmed by the hospital with no significant abnormalities):
(a) Sinus rhythm.
(b) Adult heart rate of 40-100 beats per minute (heart rate of professional athletes may be lower than 40 beats per minute).
B. Abnormal ECGs that do not meet the participation criteria include but are not limited to:
(a) Sinus tachycardia, adult heart rate > 100 beats per minute; ventricular tachycardia.
(b) Frequent sinus arrhythmia.
(c) Sinus arrest; atrial, junctional, or ventricular premature contractions or escapes.
(d) Sick sinus syndrome.
(e) Second-degree or higher degree blocks such as sinus-atrial, atrioventricular, or left bundle branch block.
(f) Pre-excitation syndrome.
(g) Myocardial ischemia, ST segment depression (including horizontal, down sloping, convex, sagging, and pseudo-ischemic patterns), T-wave changes (including flattening, biphasic, and inversion), prolonged QT interval, inverted U wave, arrhythmias, etc.
(h) Other abnormal ECG findings, such as atrial fibrillation.
(4) “Complete Blood Biochemistry” (optional, as deemed necessary)
Assessment of liver and kidney function, blood glucose, lipid profile, serum electrolytes, cardiac enzyme spectrum, etc.
(5) “Color Doppler Echocardiography” (optional, as deemed necessary)
Assessment of ventricular and atrial function, valve function, myocardial function (contractility and compliance), ejection fraction, and cardiac output, to avoid cardiac structural abnormalities.
